πŸ’Š Blog Post 4: Sacubitril/Valsartan (Entresto) – The Cornerstone of My LVSD Survival

The Powerhouse Drug: Why Sacubitril/Valsartan is the Foundation of My Care

For severe heart failure patients like me, living with LVSD (EF 20%), Sacubitril/Valsartan (often known by the brand name Entresto) is one of the most significant and effective medications in my 11-drug regimen. It is often the first major change a heart failure specialist nurse makes, and it's considered the new "gold standard" of treatment.

I take the 24/26mg dose, and this drug is a cornerstone of my survival plan.

πŸ”¬ How it Works: The Dual-Action Difference

Sacubitril/Valsartan is not one drug, but a combination of two:

  1. Valsartan: This is an Angiotensin Receptor Blocker (ARB). It helps relax my blood vessels, lowering blood pressure, and easing the strain on my heart muscle, similar to older drugs like Ramipril or Lisinopril.

  2. Sacubitril: This is the breakthrough component. It blocks an enzyme that degrades beneficial natural peptides (BNPs) in the body. These peptides help reduce blood pressure and decrease fluid build-up. By blocking the enzyme, Sacubitril boosts the body's natural defenses against heart failure.

In simple terms, this drug performs two vital tasks simultaneously: it blocks the harmful stress hormones and boosts the protective natural hormones.

⚠️ Side Effects and the Angioedema Risk

Transitioning onto Sacubitril/Valsartan often takes time as the body adjusts, and it carries two major risks that require constant vigilance:

  • Low Blood Pressure (Hypotension): Because the drug is so effective at reducing strain, it can sometimes make me feel dizzy or lightheaded, especially when standing up quickly. I must take my blood pressure regularly and report any persistent low readings to my Heart Failure Specialist Nurse.

  • Angioedema: This is a rare but life-threatening side effect involving severe swelling of the face, lips, tongue, or throat, which can block breathing. This risk is why I had to completely stop any older ACE inhibitor drugs before starting Sacubitril/Valsartan. If I ever notice sudden swelling in these areas, this is an immediate 999 emergency.

Patient Takeaway:

My Heart Failure Nurse confirmed that despite the side effects, the evidence overwhelmingly shows that Sacubitril/Valsartan provides a massive benefit in terms of longevity and quality of life for severe heart failure patients.

If you are on Sacubitril/Valsartan, know you are on the most advanced therapy available. Be meticulous about your dose timing and report any swelling or persistent dizziness immediately. This drug is fighting for every percentage point of your Ejection Fraction.
