The Warning Book: Why Dapagliflozin Demands Respect
As a patient living with LVSD (EF 20%), my Heart Failure Specialist Nurse recently added Dapagliflozin to my 11-medication regimen. While it is now a game-changing drug for heart failure (it was originally developed for Type 2 Diabetes), it came with an entire "Warning Book" yesterday—and for good reason.
This drug is a powerful ally, but managing it requires adherence to strict protocols, especially the Sick Day Rules.
What Dapagliflozin Does (The Benefits)
Dapagliflozin belongs to a class of drugs called SGLT2 inhibitors. It works mainly by making your kidneys remove excess sugar and salt from your body through urine.
Heart Protection: It significantly reduces the risk of cardiovascular death and hospitalization for heart failure.
Fluid Management: It acts as a gentle diuretic, helping manage the fluid build-up that is a hallmark of LVSD.
Kidney Support: It can slow the progression of chronic kidney disease, which is crucial for me given my multi-organ issues.
π¨ Critical Danger & Side Effects: The Sick Day Rules
The power of this drug comes with a major danger: Diabetic Ketoacidosis (DKA), a life-threatening condition where the body produces high levels of blood acids (ketones). Even if you are not diabetic (like me), this risk is real.
My Non-Negotiable Sick Day Rules:
When I am sick, dehydrated, or undergoing any medical procedure, the risk of DKA rises dramatically. My Heart Failure Nurse has given me strict instructions:
Stop Immediately If: I have severe vomiting, diarrhea, cannot drink fluids, or before any major surgery.
Monitor Closely: Watch for signs of DKA (rapid breathing, fruity-smelling breath, extreme thirst, confusion, abdominal pain). If any DKA signs appear, this is an immediate 999 emergency.
Always Check: If I am ill, I must call my Heart Failure Specialist Nurse or GP before restarting Dapagliflozin.
Other Major Side Effects I Watch For:
UTIs/Thrush: Because the drug increases sugar in the urine, it greatly increases the risk of urinary tract infections and yeast infections.
Low Blood Pressure (Hypotension): Especially when combined with other heart meds like my Bisoprolol.
